Noble Hospital to coordinate Care Transitions Education Project

WESTFIELD – Noble Hospital, in conjunction with Noble Visiting Nurse & Hospice and Westfield Care & Rehab, has been chosen as a pilot site for the Care Transitions Education Project. |
Noble Hospital will be coordinating the effort of this partnership in the development of a pilot training plan and implementation of combination didactic and experiential learning opportunities for participating nurses from each facility. Insight gathered from the implementation will be shared with the oversight group in the development of what will become a statewide curriculum for all practicing nurses and students.
The Care Transitions Education Project goal is to prepare and empower more nurses to lead effective care transitions. The three-year project (2011-2014) is designed to contribute to regional and state efforts focused on reducing avoidable hospital readmissions, improving quality patient-centered care, and reducing overall costs. The project's central strategy is to prepare the existing and future nursing workforce by creating a standard care transitions curriculum and educating nurses together across the continuum of care.
